Mid Rogue Oregon Health Plan

The Oregon Health Plan (Medicaid) provides health insurance coverage for low-income Oregonians at little or no cost. Eligibility for the Oregon Health Plan is determined by the State of Oregon. To obtain an Oregon Health Plan application, or to ask questions, call 1-800-359-9517, or TTY 1-800-621-5260. Information also is available online at www.oregon.gov/DHS.
